Replacement Cards

Does the certification expire?

Currently, Smart Serve Certification does not expire.

How much does it cost to replace my lost/ card?

Replacement cards can be purchased at a cost of $10 (HST incl).

What if my name has changed?

To order a Replacement Card, please select Login/Register*, login to your account**, select “order now” from the Lost Card bucket, and follow the prompts to attach the documents to submit the account validation request.  You will be notified by email once the account has been validated and you will then be able to proceed with the purchase.

Trouble Logging In?

* You must first Register for an online account if you:

  • completed the online training before April 2011 or,
  • took the in-class training and wrote the paper-based test

**If you have an active online account but cannot Login, please reset your password.
To update your password:

  • Click ‘Login’
  • Type your email address in the username field
  • Select ‘forgot password’ and follow the email instructions to reset your password

What if my address has changed?

You can still order a replacement card.  You will be provided with an opportunity to change your address at the time of purchase.

I don’t remember when/where I took the training?

You will not be asked to provide this information.

Where do I find my certificate number?

Once you are signed in to your online account, you should see your certificate number in the top right hand corner of the page underneath the Training Status: Smart Serve Certified.

If you do not see that information, please give us a call to ensure we have a record of your certification and can input this information into your online account.

How can I pay for the Replacement Card?

We accept the following forms of payment: Visa, Visa Debit, MasterCard, American Express, and other pre-paid credit cards.

How long does it take to receive my Replacement Card?

Please allow 7-10 business days for your card to arrive.

Can I order a Replacement Card by phone?

No, we are unable to accept orders over the telephone.

Can I still work without my card if I know my certificate number?

The certificate number alone is not considered as sufficient proof of certification; you are expected to have your wallet-sized certificate with you at all times while working.
